Insurance information is obtained when you call to schedule your first visit. Benefits are verified prior to your initial arrival. Treatment proposals are provide to each patient following your evaluation. Patient financial obligations for services are estimated based on insurance benefits provided by your insurance company. Insurance companies will not guarantee payment or the accuracy of the information provided to us, therefore, patient portions must be estimated. We make every effort to accurately determine patient responsibility. Please make certain your insurance information is accurate and up to date. We appreciate your assistance.
